Physician Case Manager
The Opportunity
The Physician Case Manager role is to assist patients in negotiating their medical journey including, developing questions to ask experts, preparing their medical information for assessment by experts, and regular continuing contact with patients to assist through the process.
Responsibilities

- Obtains a comprehensive health history by leading members through a systematic and dynamic intake assessment to capture all relevant data about their current condition and health history.

- Directs patients to the most appropriate service line based on their medical condition; sets appropriate member expectations for each process.

- Works in partnership with the clinical team in support of case development.
Support expediting procurement of patient medical records.
- Works in partnership with the clinical team in support of case development.

- Write comprehensive clinical summary and present expert physician findings.

- Communicate with patients in a caring and compassionate manner; communicate with EMS team in a courteous and professional manner.

- Track all communication with members in Salesforce

- Adhere to and abide by all regulations, policies, work procedures and instructions.
Candidate Profile

- Licensed Physician (MD/DO)
-
- Board certified in specialty/subspecialty (ABMS or AOA)

- Possess excellent written and verbal communication skills
-
- Demonstrate effective organizational and time management skills

- Able to work independently and be flexible

- Maintain confidentiality

- Proficient in technology, including Microsoft Office Suite and SalesForce
